Case study · Regional Health voice-triage deployment

From 31% voicemail rate to 6% in 4 weeks. Vapi + Claude on top of Epic. What worked, what almost killed it.

“The 4-week deployment that almost died on the HIPAA review.”

In Q1 2026 a regional health system in the Mid-Atlantic was losing patient calls at peak hours. Their front desk took 84 calls on an average Tuesday; 31% of those went to voicemail. That's lost revenue, lost trust, and lost outcomes.

The intervention: a Vapi voice agent backed by Claude, integrated with Epic for chart lookup and Salesforce for CRM. 4-week deployment, 2-person team (me + a clinical informaticist). Total project budget: $180K all-in.

Week 1. Scoped the call types — appointment booking (47%), prescription refills (22%), test result questions (16%), miscellaneous (15%). Built the call-tree taxonomy with the clinical informaticist. Vapi + Claude prototype handling 'appointment booking' only.

Week 2. Epic integration via FHIR. Started the HIPAA review in parallel — and it almost killed the project. The hospital's compliance officer wanted on-prem, which would have meant a different architecture. We ended up landing on a BAA with both Vapi and Anthropic + a private region for the LLM calls. Lesson: do the compliance work in week 1, not week 2.

Week 3. Expanded to prescriptions and test results. Voice testing with 6 patient volunteers. Bug fixes mostly around the warm-transfer-to-human pattern.

Week 4. Gradual rollout — 10% of inbound, then 50%, then 100% by Friday. Live monitoring dashboard for the clinical lead.

Outcome. Voicemail rate dropped from 31% to 6% in the first week. NPS lifted +22 points within a month. Total cost saved (in front-desk hours redirected to higher-value work): ~$340K/yr against the $180K project cost.
